TO ORDER BY MAIL OR EMAIL:
- Please print this page out & collect all the necessary information pertaining to your order to keep for
your records.
- Enter all the necessary order information into the 'Contact Us' Message box, including Your Retail Store or
Company Name, Shipping Address, Mailing Address, Email Address, Sales Tax ID, Employer ID#, and
Telephone Number.
- Click on the 'Submit' box to send us your order. We will process your order as soon as we receive it.
- Mail your completed order form to us along with the REQUIRED 50% MINIMUM DEPOSIT. Be sure to include
shipping charges and PA Sales Tax (if you are in the State of PA).
- Your order will be shipped to you as soon as we receive your deposit and payment has cleared your bank.
Bank Teller Checks and Money Orders will be accepted and will speed up the process because they clear
immediately.
- Your shipment will include an Invoice for the balance due if full payment has not been received.
Customers - Please be sure to include your Name, Complete Shipping Address, Complete Billing Address (if different), Phone Number including Area Code, & Email Address.
Wholesale Customers - Please email your order to us and wait for a Wholesale Priced Invoice from us before paying for or mailing your order with minimum deposit ! You will need special pricing - the shopping cart is only setup for Retail Customers at this time !
Please be sure to include your Store or Studio Name, Contact Name, Complete Shipping Address, Complete Billing Address (if different), Phone Number including Area Code, Email Address, EIN Number & Sales Tax Number. Sales Tax Number must be included to get wholesale pricing.

To protect the buyers and the seller we require shipping insurance for all orders. The insurance is issued by our company through the United States Postal Service at the above rate. Damaged or lost packages will be replaced at our cost or you will receive a full refund.
This webstore is only open to U.S. customers. If you are outside U.S. and want to order from us, please contact us. We only take wholesale international orders over $ 300.00. All international orders are required to pay 4% of total as shipping surcharge.
